    1. Photo of serial number

    2. Photo showing thumb and finger holes (if bowler does not use thumb please upload photo of finger holes)

    3. Photo of defect

    For Bags, Shoes, and Accessories:

    1. Photo showing top and side of product for model identification

    2. Photo of defect(s)

    For all products:

    1. Receipt or other proof of purchase. You may submit without this information however, in some cases the inability to provide this information may effect the claim outcome. 

    If you do not upload the required photographs the processing of your claim may be delayed.
